Prior to your appointment
To ensure a productive consultation, please complete the Client Health Assessment Form prior to your appointment. If the form is not submitted in advance, we will need to address it during our consultation, which may limit the time available for discussing your personalized plan.
In-Person Consultations: you may bring your completed and signed form with you to your appointment.
Virtual Consultations (online): please submit your completed form to herbingheals@hhformulations.com at least 24 hours in advance of your appointment.
Please note that if you choose to submit the form via email, you are agreeing to sharing your personal and medical information electronically.
Please Note Our Cancellation Policy
Appointments may be cancelled by phone, contact us, or email to hhformulations.com, at least 24 business hours in advance. A $30 charge will be incurred for those not cancelled within that time frame.
